CAP

The Community Assistance Program is a Ministry of Employment and Income Assistance (MEIA) funded program designed to improve the quaility of life and increase community participation for multi-barriered individuals.

CMHA Kootenays offers CAP to individuals residing in the West Kootenay communities.

You are eligible to apply for services if you are:

  • Expected to work, but only if you have significant barriers to independence and are not appropriate for other employment programming;
  • Expected to work - medical condition, but only if you have sigificant barriers to independence and are not appropriate for other employment programming; a person with persistent multiple barriers (PPMB);
  • A person with disability (PWD) who cannot access or are unsuitable for employment program for persons with disabilities; or
  • Persons that are not MEIA clients and who meet eligibility criteria established by MEIA.

What services can I expect:

  • An in-depth assessment - to determine the strength and barriers the client faces and to develop a personal plan. The plan outlines goals and the steps to be taken to achieve enhanced personal skills and increased community participation.
  • One-on-one support and referral/navigation - includes personalized coaching, reviewing of the personal plan, issuing participant support (if applicable) and proactively being linked with external services and the community.
  • Individualized services - includes personal counselling and life skills training.
  • Individual services and one-on-one support - may take a minimum of 6 months to a maximum of 12 months.
  • Follow-up services - development of a transitional plan to assist you with services upon completion of the program. Follow-up services resume for six-months.

Program Outcomes :

  • Increased community involvment.
  • Increased networking with others who may have similar barriers.
  • Commitment to healthier lifestyle.
  • Enrolment in education and or training; if appropriate.
  • Volunteer placements resulting in participation the helps strengthen ties between the individual and the community.
  • Inprove personal budgeting abilities.
  • Movement into employment programming.
  • Development of skills that contribute to stronger family relationships.
